The Governor signs each Washington State Heirloom birth certificate individually. For this reason, it can take up to 8 – 10 weeks for you to receive an Heirloom Birth Certificate after we receive your order.

What is a noncertified informational copy of a birth record?

Noncertified informational copies of birth records do not get issued on the certified paper with security features. They cannot be used for legal purposes. It will contain the same information as a birth certificate and a watermark stating “Cannot be used for legal purposes. Informational only.” Please check with the agency or organization requesting the record before ordering an informational copy.

Who qualifies?

Anyone can request a noncertified informational copy of a birth record. You do not need to provide proof of identity and proof of a qualified relationship to the subject of the record.

In order to obtain a Certified Copy you MUST complete the sworn statement included with the birth certificate application form, sign the statement under penalty of perjury and, your sworn statement must be notarized. Confidential Information on Birth Record

The T​he bottom portion of the birth certificate (entitled “Confidential Information for Public Health Use”) contains confidential personal information (race, occupation, and medical data – including pregnancy history). Health and Safety Code Section 102430 allows this confidential information to be included on an authorized copy only when specifically requested by the person named on the certificate, the parent who signed the certificate,

or if no parent signed the certificate, the mother (a father may only receive a confidential copy if he signed the child’s birth certificate). Because a confidential birth certificate contains very personal information, it is not commonly used (or needed) for identification, travel, school, or sports-related purposes. If requesting a birth certificate that contains the confidential information, please write “Confidential Copy” on the application form.​ For Mail-In Requests only.

Long Form Birth Certificate

Most comprehensive certified birth record. It is a copy of the original birth certificate. It will also show a history of corrections that have been made to the birth record.

Long form birth certificates are required to obtain:

  • U.S. Passport
  • Driver’s license (in most U.S. states)
  • Dual citizenship

Short Form Birth Certificate

A certified abstract of the birth record. This birth certificate will only show current information for the individual’s name, date of birth, place of birth, sex, and name of parent(s). This form will not show a history of corrections.

Short form birth certificates can be used for:

  • School and/or sports registration
  • Employment requirements
  • Insurance requirements

Heirloom Birth Certificate

Heirloom birth certificates are abstracts of the birth certificate. They are primarily used for framing and display. The information on an heirloom birth certificate is similar to a “short form.”

Birth Certificate to Obtain Election Identification

Vital Statistics waives the fees charged to obtain a certified copy of a birth record used to request an Election Identification Certificate (EIC) issued by the Texas Department of Public Safety (DPS). The birth record will be marked “For Election Identification Purposes Only.”

The fee waiver does not apply to nor change any other requirements to obtain a non-EIC certified birth record.

Birth Verification

A birth verification letter includes the individual’s name, the date of birth, and the county where the birth occurred. Verification letters are available for births that have occurred since 1903. Verification letters are not considered legal substitutes for certified copies of birth certificates. Vital Statistics strongly recommends that applicants ensure a verification will satisfy its intended use.

Remarks: Florida birth records less than 100 years old are confidential by law. The Bureau of Vital Statistics has limited birth records dating back to 1850. The majority of records date from 1917. The fee is $9.00 for the first year searched and includes one certification of the record. Additional copies are $4.00 each when requested at the same time.

If no record is found, a certified “No Record Found” statement will be issued. If the exact date of birth is unknown and additional years need to be searched, a fee of $9.00 for the first year searched and $2.00 for each additional year (up to a maximum of $50.00) will be charged.

Current Year Corrections (CYC): Current year corrections are defined as corrections made to the certificate within one year of the child’s birthday. There is no charge for current year corrections.

General Amendments: A general amendment is defined as a change that is made to a certificate after 1 year of age. Amendments can occur due to a legal name change, adoption, legitimation, adding of a spouse and/or paternity acknowledgement.

Eligible Recipients

Birth Certificates are only issued to applicants having a direct and tangible interest to the registrant, i.e. immediate family members, and/or legal representatives of the family.

There are instances in which specific documentation is required based on who is requesting the record.

  • The person named on the certificate- If the person named on the certificate (i.e. the registrant) is the requestor, that person must provide valid photo identification at the time of the request.
  • The parent(s) named on the birth record- Must provide valid picture identification.
  • An authorized legal guardian or agent- Any person who has legal custody or control of a minor child must provide a certified copy of the court order establishing guardianship and legal custody.
  • Grandparents of the person named on the certificate- Must provide proof of relationship such as the birth certificate of the registrant’s parent.
  • An adult child or adult sibling of the person named on the certificate- Must provide proof of relationship by providing a copy of his or her birth certificate listing one of the same parents, along with his or her valid government issued picture identification which includes signature.
  • The spouse of the person named on the certificate- Must provide a copy of the marriage certificate, a photocopy of the spouse’s picture identification, which includes the spouse’s signature, with a notarized letter from the spouse giving permission.
  • Attorney- Must represent an immediate family member and provide a notarized letter on letterhead signed by the attorney; provide bar number indicating reason for the request and whom they represent; provide supporting documentation with the fee; provide a notarized release from the biological mother, in the event of an adoption.
  • State or Federal Government Officials- The State Registrar or the local custodian may disclose data from Vital Records to authorized representatives of Federal, State, or County agencies of government which request such data in the conduct of their official duties.

A copy of a valid photo ID and a signed application are required for all certificate requests. See NC Vital Records’ certificate application for a list of acceptable IDs.

Birth Certificates

The South Carolina Vital Records offices issue birth certificates for births that occurred in South Carolina since 1915.

You may request a certified copy of a birth certificate if you are:

  • The person named on the certificate (if you are at least 18 years old),
  • The parent(s) named on the birth certificate, or
  • The legal guardian, or a legal representative of one of the above.

Immediate family members and/or their respective legal representative may obtain a certified copy of a birth record if the registrant is deceased. An original, certified copy (no photocopies) of the registrant’s death record must be submitted with the request.

Newborn birth certificates are not automatically mailed to parents. Information gathered in the hospital is solely for the purposes of registering the birth. The hospital will not order your child’s birth certificate for you. You must request the certificate by one of the methods below.

There are two types of birth certificates:

  • Short Form: The short form is the most commonly requested certificate. It can be used for most domestic purposes. This form includes your child’s first and last name, place of birth, date of birth, sex at birth and mother and father’s first and last names.
  • Long Form: The long form includes additional information and is generally used for international purposes, such as dual citizenship, international adoption, international marriage, time of birth and apostille certificates.

If you are unable to schedule an appointment and have an emergency request related to travel, health care coverage, government services, military, housing or employment, call 311 or email nycdohvr@health.nyc.gov. In your email, include current photo ID and documents showing that you have an emergency, such as a paid plane ticket, letter from an employer with a job start date, or letter from a government agency with an appointment date.

New and Expectant Parents

Parents receive a free birth certificate for newborns about four weeks after birth. Your baby’s birth certificate will be based on information you provide at the hospital when you deliver. It will be automatically mailed to the parents listed on the certificate.

At the hospital, you will be asked to fill out a Mother/Parent Worksheet (PDF). You can complete this ahead of time and bring it to the hospital or give it to your midwife. The worksheet collects important information and helps your baby get a Social Security Card.

Be careful when you fill out your baby’s full name and your mailing address. Correcting a birth certificate can cost money or even require a trip to court. A mistake in the mailing address can delay delivery.

Married parents, regardless of sex, have the right to have both their names on their child’s birth certificate. Some hospitals require proof of marriage. Check with the hospital to find out if you need to bring a copy of your marriage license with you.

An Acknowledgment of Parentage is a document that unmarried parents must both sign voluntarily to establish the child’s legal father/parent.

Certified copies of an Acknowledgment of Parentage are free of charge. Complete the New York State Acknowledgment of Parentage application and submit by mail or in person.

There is no fee to add a child’s given name to a birth certificate if you submit the application within 60 days of birth.

You may add a given name and submit corrections if you find missing or incorrect information at the hospital. There is no fee for changes made by the hospital if sent within 12 months of birth.
